You can include details about the spam (i.e. if it offended you or if it was pornographic in nature) in your complaint too. If you are going to follow up with law enforcement or pursue this further, include those statements in the e-mail as well.

If you REALLY want to complain to an ISP, find out the ISP's phone number and call them to log a complaint. They will most likely be more tuned in to those calling with complaints than the thousands of complaints they receive via e-mail. Make sure you have the full Internet headers and spam message in front of you when you call.

If you use any of the templates below, make sure you tailor it to your specific complaint:

This can be used when complaining to an ISP or mail provider that was used in sending the spam to you.
Dear Sir/Madam,

Below you will find the body and full Internet headers of an unsolicited [pornographic/offensive] e-mail I received through your service. I am forwarding it to your attention as I am sure that you will want to take the appropriate actions against your client and keep your good name as much as I do. I'm sure your acceptable use policy prohibits this type of action.

Thank you for your prompt attention to this matter,

<Your Name>

... include Internet headers and body of content...

 

This can be used when complaining to an ISP or business that supports the content of the spam (i.e. by hosting the website of the content of the e-mail or allowing the spammer to be in business).
Dear Sir/Madam,

I'm sure you are interested when your company's name is associated with [forged/pornographic/offensive] e-mail spam sent out to users not requesting such information. Even though the e-mail may not be coming from your network, your site and company's name is being used in the content of the spam and it is quite [disturbing/annoying/offensive]. Your company allows this spammer to continue to be in business which means there is more unsolicited [pornographic/offensive/annoying] spam tying up the Internet.

I'm forwarding it to your attention as I am sure that you will want to take the appropriate actions against your client and keep your good name as much as we do.  I've attached the body and full Internet headers of the unsolicited e-mail message below.
